Ultrasound-Guided Filler · Newport News
Filler in Newport News, placed while watching the anatomy, not from memory of it.
At Peninsula Aesthetic Medicine in Newport News, Virginia, filler in the temple, the tear trough and under-eye, the nose, the nasolabial fold, and any area with previous filler is injected under ultrasound guidance, so we can see the vessels while we work. Guidance carries no surcharge; it is included in the price of the filler. Adam Medlin, FNP-BC, performs every treatment himself, using a Clarius L20 high-frequency handheld ultrasound.
Anatomy varies between people
The textbook shows where a vessel usually runs, not where yours runs. In most of the face that variation changes little. In a few areas it changes everything.
Ultrasound shows where your vessels actually run, before and while we inject. The device is a Clarius L20, a high-frequency handheld ultrasound.
The temple
The superficial temporal artery and the middle temporal vein both run through the temple, and the middle temporal vein is wide and thin-walled. The published complication list for temple filler includes vascular occlusion, skin necrosis, blindness and stroke [1].
Injecting the temple under ultrasound guidance — into the interfascial space, with the cannula and the filler visualized in real time — is described in the peer-reviewed literature [1].
The other areas we inject under guidance
- Tear trough and under-eye. The skin is thin and the vessels sit close to where filler goes. Depth matters here, and ultrasound shows depth.
- Nose and nasolabial fold. Vessels here connect toward the circulation of the eye, which is why these areas appear in published reports of filler-related vision loss.
- Any area with previous filler. We see what is already there, and where, before adding to it.
Elsewhere in the face, guidance is available on request.
What the evidence does and does not show
Ultrasound-guided filler injection is described in the peer-reviewed literature [1], and the anatomical rationale is strong: anatomy varies, and ultrasound shows the anatomy that is actually there.
What does not exist is a trial comparing complication rates with guidance versus without. The published work is mostly descriptive — techniques and case series. The controlled comparisons that do exist measure minor outcomes such as bruising [2], not vascular occlusion, skin necrosis or vision loss.
Anyone claiming ultrasound guarantees a safe result is overselling it.
No surcharge
Guidance is how we do these areas, so it carries no surcharge. You pay for the filler, which is priced by the syringe and published product by product on the dermal fillers page.
If you want a scan and an opinion before deciding anything — particularly about filler placed somewhere else — a diagnostic filler ultrasound is $150, credited in full toward any treatment booked at that visit. Questions
Does it hurt more?
No. The probe sits on the skin with gel; it adds nothing to what the injection itself feels like.
How much longer does it take?
About 15 to 20 minutes more than the same treatment without it.
Can I have it if my filler was placed elsewhere?
Yes — that is when it is especially useful. We can see what is already there before adding to it.
Do you use it for every filler treatment?
The high-risk areas — the temple, tear trough and under-eye, nose, nasolabial fold, and anywhere with previous filler — are done under guidance as standard. Elsewhere it is available on request.
What machine do you use?
A Clarius L20, a high-frequency handheld ultrasound. There is more about it on our equipment page.
